Short-Term Services
Our Short-Term Services are designed to facilitate a safe and comfortable return home after hospitalization or treatment. Each client receives a personalized care plan tailored to their specific health needs, promoting a smooth recovery while fostering independence within the comfort of their own home. Our team provides a wide range of support services, including:
- Post-Operative Assistance: Help with surgical recovery, wound care, and safe mobility to aid in a smooth transition to home life
- Heart Attack Recovery: Lifestyle support and education to encourage heart-healthy habits and enhance recovery
- Diabetes Management: Regular foot inspections, blood glucose monitoring, and family education to assist with daily diabetes care
- COPD Support: Guidance on inhaler use, medication reminders, wellness coaching to improve lung health and overall well-being
- Pneumonia Care: RN medication management, oxygen therapy support, and personal care support from HHA to promote comfort and recovery
- Stroke Recovery Support: Assistance with activities of daily living (ADLs), fall prevention, medication reminders, and transportation to appointments for physical, occupational, or speech therapy
In addition to our medical services, we also offer Personal Care Assistance, Homemaker Services, and Night Supervision to support clients in their recovery:
- Personal Care Assistance (PCA): Assistance with daily activities such as dressing, grooming, bathing, toileting, and mobility
- Homemaker Services: Support with light housekeeping, meal preparation, laundry, and home management tasks
- Night Supervision: Awake and overnight care
- 24-Hour Emergency Assistance: This service offers immediate access to help for health or personal emergencies, anytime, day or night. Our team provide counseling, guidance, and problem-solving, and will dispatch assistance to the client’s home if required. With 24-hour emergency support, clients and families can rest assured that timely and compassionate care is always available when it matters most
