At Oasis Home Health Care, we provide Individual Community Living Supports (ICLS) to help individuals maintain their independence and live comfortably in their own homes. This service provides assistance tailored for individuals who need reminders, cues, moderate supervision, or physical support to thrive in their daily lives.

What Is ICLS?

ICLS is a bundled service that includes six essential components, offering assistance and support for adults who need reminders, cues, intermittent/moderate supervision, or physical assistance. This service helps individuals maintain their independence and continue living in their homes.

ICLS provides assistance and support in the following six service components. To be eligible, individuals must receive support in at least two of these areas:

1. Active Cognitive Support

Supporting individuals with cognitive challenges to address concerns and maintain orientation.

  • Problem-solving daily living concerns
  • Providing assurance and guidance
  • Observing and redirecting behavioral concerns
  • Conducting check-ins to resolve issues

2. Adaptive Support Service

Encouraging self-sufficiency and reducing reliance on human assistance.

  • Providing verbal, visual, or touch guidance for task completion
  • Developing and demonstrating reminder tools (e.g., calendars, lists)
  • Assisting with understanding assistive technology
  • Practicing strategies to maintain independence

3. Activities of Daily Living (ADLs)

Helping individuals perform essential daily tasks.

  • Providing reminders or cues for ADLs
  • Offering intermittent physical assistance with dressing, grooming, and mobility
  • Providing supervision and assistance with bathing as needed

4. Household Management

Assisting with maintaining a clean, organized, and functional home environment.

  • Cleaning, meal planning/preparation, and shopping
  • Budgeting and money management
  • Managing communication (e.g., mail, phone calls, scheduling appointments)
  • Providing transportation integral to household management goals

5. Health, Safety, and Wellness

Supporting overall well-being through proactive and responsive care.

  • Identifying and addressing changes in health needs
  • Coordinating environmental safety improvements
  • Providing medication assistance (e.g., refills, reminders, administration)
  • Monitoring health as instructed by licensed professionals
  • Using adaptive technology as prescribed

6. Community Living Engagement

Helping individuals participate meaningfully in their communities.

  • Facilitating access to community activities and services
  • Developing and maintaining informal support systems
  • Providing transportation for community engagement goals
